PHCC announced Case Management Services at all health centres in Qatar

As part of Phase 2 of the Family Medicine Model, Qatar's Primary Health Care Corporation (PHCC) has announced the expansion of its case management services to all of the nation's health institutions. After a trial project at the health clinics in Umm Salal, West Bay, Al Wakra, and Al Wajbah was successfully completed, the service was implemented. As it supports the integrated family medicine paradigm of treatment, the launch of the Case Management Service is seen as a major milestone for PHCC.

Case management is a widely accepted paradigm that has shown to be very successful. It is intended to give complete and ongoing care to patients with numerous chronic illnesses and complicated care demands.

The Case Management Service at the PHCC is an integrated model of care that unites a diverse group of medical specialists. To enhance patient outcomes, particularly for individuals with comorbidities, this team will collaborate to deliver individualised care plans, care coordination, and case management. This will guarantee that patients receive comprehensive and patient-centered care.

In addition to ensuring that patients receive the right care and can effectively manage their chronic diseases, the case management service will play a critical role in fostering continuity of care, lowering hospital admissions, and enhancing patient safety.
